Hair Transplantation in General

Hair Transplantation

Hair Transplantation is defined as the process of harvesting follicular units from the back and sides of the scalp (donor area) and the reimplantation to the area affected by hair loss (recipient area) and it is the medical treatment for Androgenetic Alopecia (Male Pattern Baldness). Hair Transplants΄ concept is based on the principle that hair on the back and sides of the scalp is genetically resistant to hair loss, therefore it can be extracted and implanted to the thinning or balding areas of the scalp where it will continue to grow and behave as into its former position. In fact, hair transplantation is the redistribution of the existing hair of the scalp in order to achieve coverage & density at the thinning or balding areas. As the process of hair transplant reproduces the natural hair growth, it allows the accomplishment of natural results.

Today΄s most advanced hair transplant techniques are FUE (Follicular Unit Extraction) & FUT (Follicular Unit Transplantation). The basic technical difference between FUE & FUT concerns the follicular units΄ harvesting, as in FUE the follicular units are extracted one by one from the donor area, while in FUT the receiving of the follicular units is accomplished through strip excision (single strip harvesting and stereo-microscopic dissection).

After a hair transplant, a patient has to wait approximately 3 months before experiencing the first signs of hair growth. The transplanted hair will continue to grow during the following months and in 10 to 12 months (or more in some cases) the patient experiences the complete hair transplant result. As it has already been mentioned, hair transplant redistributes the existing hair from areas of the scalp with adequate density to the areas affected by hair loss. Thus, hair transplant does not create new hair and cannot stop hair loss which means that it cannot provide a full head of hair and basic prerequisite for someone who wishes to have a hair transplant procedure is to have adequate donor hair supply. The final result varies from person to person, as different parameters have to be considered in each case: the facial characteristics, number of transplanted hair follicles, shaft diameter (fine hair provide less density), hair color and texture, skin color, etc. Thus, in the case of 2 different sessions involving the same number of hair follicles in different persons, the result will be different for each.

Treatment

Our doctors establish a Personalized Hair Transplant Plan for each patient in order to provide an individualized hair transplantation that is completely aligned with the patient΄ s personal desires, characteristics, age and style.

All hair transplant techniques aim at the improvement of a client’s aesthetic appearance without any changes to the basic structures of their physiognomy. After having a hair transplant procedure, the goal is to have the best possible aesthetic appearance that is absolutely matched to the client’s profile profile.

Hair Transplantation is a painless & safe medical procedure that requires advanced skills, long-lasting experience & high aesthetic perception by the doctor. It is very important to accomplish an undetectable and refined result for the patient, since hair transplants must have a beneficial impact on his quality of life.

It lasts a few hours, with the duration of the procedure to depend on the size of the session (number of follicular units that will be placed) and on the type of the hair transplant technique that will be performed. Due to the operation protocol, time goes by pleasantly and the patient doesn’t feel any anxiety or stress during the procedure.

Hair Transplantation is performed under local anaesthesia and it is a virtually painless procedure. It is also followed by a smooth post-operative course.

Within a few days the patient is able to return to his normal life. The recovery time depends on the hair transplant technique, the skin of the patient, as well as his activities and profession. There are some restrictions that the patient has to abide by, such as weight lifting, intense physical exercise, direct sun exposure etc.

Steps

  • Hair follicles are carefully extracted and kept intact, either through microscopic dissection of the tissue in FUT or one by one in FUE.
  • Hair follicles are classified according to the number of hairs they consist of.
  • Hair follicles are protected until the time they will be reimplanted.
  • The appropriate recipient sites are created.
  • Hair follicles are transplanted to the recipient sites.

During hair follicles placement, Bergmann Kord’s doctors take into account all the necessary parameters that will define a natural result.

  • Proper direction of hair follicles so that they become fully harmonized with the growth conditions of the recipient area.
  • Appropriate distribution of hair follicles to the thinning area, in order to ensure maximum density.
  • Effective combination of all factors that allow a refined and natural frontal hairline.
  • Hairline is seen at first sight and should be a soft frame for the face and the most natural link between forehead and hair.
  • Many skills are therefore required by the doctor, not only for the initial design of the hairline but for its creation as well.

FUE

The FUE hair transplant technique at Bergmann Kord Hair Clinic involves the one by one extraction of follicular units from the donor area. It is implemented through the extraction of follicular units from the back and sides of the scalp and their reimplantation to the area affected by hair loss. The FUE can reproduce natural hair growth, as transplanted hair regrows in its natural occurring groups of 1-4 hairs. The FUE method has eliminated the need for a linear incision (no scar technique) and allows hair transplant doctors to perform Follicular Unit Hair Transplantation through single and direct harvesting of the follicular units. The FUE process involves a circular incision (of 0.7mm-0.9mm diameter) around each follicular unit, the extraction of the follicular unit and its implantation to the balding area of the scalp. The FUE method enables physicians to harvest individual follicular units, while ensuring the minimum of peripheral damage.

Basic Information about FUE

  • FUE can be implemented for men & women with Male or Female Pattern Baldness (Androgenetic Alopecia)
  • The surgeon can exploit the back & the sides of the scalp for donor supply
  • FUE is less invasive than FUT, due to absence of linear incision and thus is associated with a smoother postoperative course (the donor area requires a shorter healing period and it is left to heal by second intention)
  • FUE is usually preferred by people who tend to keep their hair very short (less than 1cm long)
  • FUE is ideal for hairlines, temples, sides, eyebrows & scar repairs
  • The presence of imperceptible small white spots allows quite short haircuts, but in cases of extensive extraction of follicular units scars may be noticeable
  • FUE is a time-consuming method and for the same number of follicular units it requires more sessions than Strip (and It is more expensive per price/follicular unit)
  • Mega FUE sessions (more than 2.500 follicular units) may have to be completed in 2 subsequent days (this is usually defined after your personal consultation & examination by the doctor, as it depends on personal parameters)
  • There are specific preconditions for being an FUE candidate (limited donor supply, limited scalp laxity – very tight scalp, skin that tends to heal scarring etc)
  • FUE is also preferred when someone has undergone several previous hair transplant surgeries and is no longer able to undergo a further Strip procedure

What you should know before the FUE

There are two ways to proceed with the preparation needed prior to the Follicular Unit Extraction. The first way requires that the patient has a marine haircut, and then the team of hair transplant doctors proceeds with the extraction of follicular units one by one, from the donor area. This requires though that the patient will not be able to return to his work and usual activities for at least 15 days. Additionally, shaving the head could lead to the patient feeling uncomfortable, because of the resulting change in appearance.
For those patients who do not wish such a change of appearance, or who cannot tolerate shaving due to their social status or job, or who are reluctant to shave due to the scars on the scalp, a second way of preparing the donor area before surgery is available. If a patient’s hair is long enough at the back of the head there is the option of shaving only a specific zone (or a few zones) from the donor site. This way, only a strip of about 1 inch wide needs to be shaved, and then it gets camouflaged by the rest of hair, that is long enough to cover the area until hair re-grows. This method can be so undetectable, that many patients are not able to tell their hair was even shaved.
Based on the consultation that you will have with our doctors, you can decide which method of preparation is best for you.

Brief Description of the FUE Procedure

Preparation
FUE is implemented through local anesthesia and is therefore a virtually painless procedure. Hair in the donor area will be clipped to a 1mm length.

Follicular Units’ Extraction
Our doctors, with the use of special, up-to-date surgical tools of minimum diameter, extract hair follicles one by one from the donor area. The extraction sites are -for the time being – small, red spots. Each extracted follicle comprises of 1 to 4 hairs.

Hair Follicles’ Preservation
Extracted hair follicles are kept in a NaCl aqueous solution until the time they are reimplanted.

Care Of Donor Area
No linear incision is needed with the FUE, thus the extraction sites receive simple medical care with antibiotic ointment and are temporarily covered (Healing by second intention).

Recipient Sites Creation
The doctor creates the recipient sites to the thinning areas of the scalp, taking into account all the required standards for their size, direction and distribution to the thinning area. Recipient sites should ensure proper preconditions of growth for the reimplanted hair follicles and should not disturb the elasticity and microcirculation of the recipient area.

Hair Follicles’ Placement
During hair follicles’ placement, Bergmann Kord doctors take into account all the necessary parameters that will define a natural result:

  • Proper direction of hair follicles in order to become fully harmonized with the growth conditions of the recipient area
  • Appropriate distribution of hair follicles to the thinning area, in order to ensure maximum density
  • Effective combination of all factors that attribute a refined and natural frontal hairline. Hairline is seen at first sight and must be a mild frame for the face and the most natural link between forehead and hair
  • Reproduction of the natural way of hair growth: single hair follicles are placed along the frontal hairline and as we move toward the upper side of the scalp, the doctor reimplants hair follicles with 2, 3, or 4 hairs

Donor and Recipient Area Examination
At the end of the procedure, the doctor will examine the donor and the recipient area. You will be given all the necessary written and verbal post operative instructions and then you will return home.

24h Medical Care
In the days following your procedure, Bergmann Kord will be by your side, providing you with round -the -clock medical care.
The first signs of hair growth will be experienced 3 months later, while in 10 months you will have a complete aesthetic result.

The doctor should examine the patient and decide the type of the procedure that is suitable for each case. In many cases, a combination of both FUT and FUE techniques attains the desirable result.

FUE for eyebrows, mustache and beard

FUE for eyebrows

The eyes are the most important feature of our face. What is often forgotten though, is that the eyebrows which “frame” our eyes, are of equal importance. Many people have thin eyebrows or no eyebrows at all. In some cases, women may have used the tweezers too abusive, and as a result their eyebrows appear extremely thin. In such cases FUE can be performed in order to replace eyebrows hair follicles and produce natural and beautiful results. Hair follicles are taken from the donor area, which is usually the back of the head, and get transplanted into the eyebrows. Each hair follicle is removed one by one, and as a result there are no stitches and healing is faster.

FUE for mustache and beard

In many cases, a patient may require extra hair in the facial region, as a result of scarring, uneven growth or a desire to have a greater density at the beard and mustache. FUE can be performed to these areas, with great and undetectable results.

FUE for scars

Follicular Unit Extraction (FUE) is a great method to correct or conceal scalp and facial scars caused by burns or accidental trauma. The smaller the scar, the easier the procedure will be. It should be noted that the scar tissue will remain in the region; however the newly transplanted hair will grow normally, giving a natural and undetectable result. This way it will simply be impossible to detect the scar. FUE has become increasingly popular with people who suffer from facial or scalp scarring.

PRP (Platelet Rich Plasma)

PRP (Platelet Rich Plasma) hair loss treatment, which is an innovative treatment method for male and female hair loss. PRP is a method of autologous tissue revitalization and regeneration and is based on platelet rich plasma, a portion of blood enriched in platelets that is easily extracted through our special protocol, from a small blood sample. The direct injection of PRP into the area affected by hair loss appears to be highly effective as a hair loss treatment as well as a hair transplant boosting treatment. This method takes advantage of the various growth factors that platelets contain, which promote cell proliferation and tissue regeneration.

The role of Growth Factors in hair loss treatment

Two of the most important components of PRP are Cytokines (proteins of growth differentiation and immunological response) and Growth Factors. Growth Factors are special proteins which activate the repair of tissues which do not function properly or have been damaged. The Growth Factors promote capillary formation (angiogenesis), tissue repair and collagen production. They also encourage the normalization of the hair follicle unit. Evidence also suggests that PRP can reverse the miniaturization process of hair follicles (a result of DHT dehydrotestosterone), when injected into the balding area of the scalp. Growth factors reactivate the reactivation of the weak adult stem cells and the proliferation and regeneration of the cells of the area of concern.
Based on the fact that platelets contain various growth factors, through PRP therapy the concentration of growth factors is greatly increased, and thus a profound number of adult stem cells are reactivated. They then give the appropriate signals for the refunctioning of the area of concern. PRP is able to reactivate the impaired hair follicles that do not operate properly in order to provide invigorated hair. PRP “forces” the human organism to react in an intense but completely natural way to the impaired function of hair follicles by supplying the area of concern with activated cells which greatly contribute to the regeneration of hair follicles.

Is PRP a safe hair loss treatment?

The autologous nature of PRP renders it an absolutely safe treatment with no side effects. Furthermore PRP is a simple and painless process implemented by Bergmann Kord΄s scientists with strict adherence to hygiene & safety regulations. Moreover, PRP is itself an antimicrobial & anti-inflammatory biological material and therefore it provides antiseptic action and protection.
PRP treatment has been implemented for years in wound healing (orthopedics, chronic ulcers, burns and surgical scars) with very impressive results.
Contraindications: they are few and related to unregulated blood sugar or anticoagulant treatment. Before any PRP treatment, the doctor carefully reviews each patient’s medical history.

In which cases do we implement PRP?

  • PRP treatment is implemented for men & women who suffer from hair loss at early or advanced stages:
  • For the treatment of Androgenetic Alopecia
  • As a supportive treatment to Hair Transplants
  • For the treatment of Alopecia Areata
  • For the prevention of hair loss

PRP results
Significant results have been observed in hair loss treatment, mainly due to the fact that hair loss can be reversed and the result can be preserved. Nevertheless, we should mention that there is a small percentage of patients to whom the PRP treatment has slight or even no effectiveness.

How often should the PRP treatment be offered?
The frequency for the PRP treatment depends on each case, and it is usually repeated every 6 months or 1 year. Nevertheless, in cases of Alopecia which require more intense treatment, it should be repeated more frequently.
